Our Swing Bed Unit consists of 6 newly renovated rooms on a rehab wing dedicated to providing compassionate care to patients who are in need of transition assistance after acute hospital care. Our caring team ensures that each of our patient’s Therapy Plans and Plans of Care are tailored to their individual needs.

To provide quality and compassionate healthcare to patients who no longer require acute care, but are still in need of services to transition back into their community and return home.  We are dedicated to promote optimal health and independence through an inter-disciplinary approach.  Our commitment is to help patients achieve their highest functional level possible.

What is Swing Bed?

Swing bed is a program that allows patients to receive skilled care services once acute hospital care is no longer required, but the patient continues to need services that cannot easily be provided in the patient’s home.

What Services Are Included?

The program is ideal for patients who require physical therapy, IV medication therapy, wound care, nutrition services, patient education, social services, or other care services.

Who Is Eligible?

Swing bed is available to patients who have been hospitalized.  Swing bed admission requires you to have stayed at least 3 consecutive days (3 midnights) or more in any acute hospital.  You have skilled needs which are related to the acute admission within the last 30 days.
